Sometimes we even introduce a working Saturday so that trauma patients wouldn’t wait for surgery in an attempt to make up for everything we lost while in the hospital twice. Of course, it can’t make up for it all, but we do at least 30 surgeries a day. And we hope it is not necessary to re-enter the kovid system. And that will depend exclusively on the behavior of citizens. If they are responsible and respect all epidemiological measures, we will not have to enter the covid system for the third time, says Professor Dr. Zoran Bascarevic, acting director of the Institute of Orthopedics “Banjica” in an interview for Kurir.

How many patients were in “Banjica”?

– On both shifts, and we were on the kovid system in April and July for six weeks, we had about 800 patients. There were days when we had 300 patients in the hospital, and more than 150 of them on oxygen, that is, with some kind of oxygen support. At one point, we had 16 patients on a ventilator, which is the largest number of them simultaneously on mechanical ventilation. I must also highlight the great support of the internists of the Serbian Clinical Center, as well as the endocrinology, hematology, pulmonology clinics. Up to 40 doctors came to our aid.

At one point, his operating rooms were busy.

– In the second wave, in July, in addition to filling the intensive care unit with the most difficult patients, we also arranged a part of the operating room to house the seriously ill. There we had a convenient place to distribute gases, install respirators. We had modern respirators and received up to 16 new ones from the state, which we implemented where we could do more intensive treatment. The second wave had a much more severe clinical picture than the first.

What surprised you?

– No more surprises. Kovid 19 is a serious respiratory infection that endangers the life of the patient. It attacks his lungs and he is unable to fight for his own breath. We consume more than five tons of oxygen per day, which is three times the consumption of “Banjica” under normal circumstances, when we function as orthopedists.

You yourself went through the kovid 19. When and what was the clinical picture?

– In “Banjica”, a total of 20 of us tested positive for coronavirus, out of 350 employees. Fortunately, no one got seriously ill and they were all, so to speak, household infections. No one got infected at work. We had great protection at work. Our instrumentalists dressed and undressed all employees who entered the red zone. No one could be infected by an infectious suit or because he was not dressed properly. The internists especially thanked us, because until now they had not used spacesuits and did not know how to handle them. And none of them got infected at Kovid hospital. I had kovid 19 in July, the clinical picture was easier. However, I still had to stay for a few days in the Infectious Disease Clinic.

And the consequences of the covid?

– And when you have officially recovered and are negative for the coronavirus, weakness, weakness, deconcentration and fatigue – stay. You can no longer work eight or nine hours a day without any problem, with enthusiasm, but after two or three hours you are dead, which I myself felt. Fortunately, I now live and work normally.

Now “Banjica” is running at full speed?

– Yes, everyone at full steam. We left the kovid system on August 26 and took the first ambulance on August 31. We carry out three emergency services a week: Monday, Thursday and Friday. We have between 50 and 60 trauma patients a week. We operated on all malignant patients whose treatment had to be postponed. We do both kids and waiting list, we install 10 artificial joints every day. Of course, we also do the spine. We’re back to full workload, but unfortunately, you can’t make up for lost time. Sometimes we even introduce a working Saturday so that trauma patients don’t wait for surgery. We carry out at least 30 operations a day. Every day we have eight operational programs: two for trauma, one for malignant diseases, three for waiting lists, and at least two more operational programs for all other conditions, including spinal deformities, plastic surgery, infections.

Are you afraid of the next peak of the crown?

– I wouldn’t say we’re scared. We do not expect “Banjica” to have to go kovid again, and we are working as if that is not going to happen. But even if it happens, it does not depend on the will of anyone, but exclusively on the citizens. If they are responsible and respect all the epidemiological measures of the crisis headquarters – wear a mask, keep a distance, constant and regular disinfection and meticulous personal hygiene and hygiene of the space – we will have “Banjica” in orthopedics. Otherwise, we will see patients from Kovid again. I certainly don’t wait, and if that has to happen a third time, let’s get into the kovid system so quickly.

AFTER LEAVING THE KOVID SYSTEM

Twice as many malignant patients

Due to the kovid, we will have an epidemic of malignant, cardiovascular and many other diseases, because many of them were “on hold”. And his patients have felt it on their own skin. Is disease progression seen?

– The Serbian Institute of Oncology and Radiology fully admitted the malignant patients, whose treatment began in “Banjica”. Since we left the kovid system, we have practically doubled the number of malignant patients. An operating room every day is only for those suffering from malignant diseases. However, we did not notice that there was a progression of the disease because we were in the covid system. Everyone who studied at that time was treated at the Institute of Oncology. These patients did not stay home. Even our oncology orthopedists went to “Tirsovo”, where children with malignant diseases were being treated, and to the Institute of Oncology. We do not introduce the doctors from the oncology department at kovid to be available to nevid patients.
