How does the crown affect the occurrence of a stroke and what determines the outcome of the treatment of such patients, as well as how many patients with severe covid are placed in the Special Hospital “St. Sava”, reveals for “Blic” the acting director of this institution, the neurologist Prim. dr sci. medicine Marjana Vukićević.

Day by day, the coronavirus breaks black records on the planet, denies many scientific theories in virology and medicine, shows a “new face” and leads to various symptoms, without forgetting the consequences of the infection.

However, it is now known that the crown not only affects the onset of severe pneumonia, the development of myocarditis, hair loss, loss of the sense of taste and smell, but can also lead to a stroke. This has been published in some scientific journals around the world, and our doctors have reached the same conclusion in their daily work with patients.

In a conversation with “Blic”, Dr. Vukićević notes that our physicians continue to learn about stroke during the kovid epidemic from their own experience, as well as the experiences of others through published work.

– In our country, patients appear during the acute phase of kovid, most often a few days after the temperature jump. However, the works show that a stroke can be related to a covid infection even after 21 days from the cure of the infection, notes Dr. Vukićević.

These are very serious strokes, she notes, and they occur most often in people between the ages of 60 and 70.

– The most common symptoms are deprivation on one side of the body and speech problems. Elderly patients also have a worse prognosis, and most of the time ends in death or severe disability. Patients with a milder picture of covid infection have a better prognosis for the outcome of a stroke, says Dr. Vukićević.

Why does the crown cause a stroke?

Our interlocutor points out that there are several reasons from which the connection between covid infection and stroke can be explained.

– Elevated body temperature, insufficient fluid intake, loss of smell and taste reduce the need for food and fluid intake, increased sweating, stomach disorders in Kovids can cause dehydration, which in the elderly can much more easily cause blood clots in the blood vessels of the brain, especially in those who already have diseases of the blood vessels and the heart – explains Dr. Vukićević.

On the other hand, says the doctor, the corona virus increases the tendency to form clots both in the arteries and in the veins of the brain.

– The virus also causes inflammation of the blood vessels, which are then more likely to become clogged. Inflammation of the heart muscle with arrhythmias often occurs, which also leads to the formation of a clot. There are many mechanisms by which a stroke can occur in patients with covid infection, which shows how important it is in the application of protection measures, the use of masks, avoiding meetings, hygiene – says the interlocutor.

Research carried out by the doctors of this hospital in cooperation with colleagues from the Serbian Clinical Center and the Military Medical Academy is ongoing, in order to determine what kind of stroke patients, and in whom the crown was confirmed, had a clinical picture during treatment, what symptoms were recorded. , how was the treatment …

– The investigation is coming to an end and took care of kovid patients in non-kovid institutions. We wanted to determine which patients they were and what medical conditions they had when the presence of the virus was confirmed, since they did not present symptoms of infection upon admission to the hospital. There were 50 such patients in our country, and then they were transferred to the institution – said Dr. Vukićević.

And your kovid capabilities are full

Since the hospital capacities of kovid institutions in Serbia are filling up rapidly, recently, more precisely on November 6, a part of the Special Hospital “Sveti Sava” entered the kovid system.

– It took us five days to equip and prepare the largest department of the Hospital with the help of epidemiologists, funds from the Ministry of Health and Hospital employees. It is a complete second floor with 60 hospital beds. Twelve days after the inauguration, our department is almost full, says Dr. Vukićević.

He adds that 10 percent of patients with a milder clinical picture are in Kovid’s ward, while the other 90 percent require intensive care.

– Ten patients require the help of mechanical ventilation and are the most complex patients. Treatment is multidisciplinary. The department coordinator is an anesthesiologist, and a neurologist, internist and specialist is present at all times. Nurses are our fighters, because with their maximum commitment, they manage to provide adequate care to these difficult patients. I hope that as a team we will be able to face the challenges that lie ahead, that is, a parallel system of treatment and provision of all kinds of treatment, stroke patients and non-cadaveric wards – says our interlocutor.

Prevention

Dr. Vukićević emphasizes that at this time, the most important thing is the prevention of stroke and the prevention of covid infection.

– It is necessary to take a regular therapy prescribed by an internist, neurologist or chosen doctor. Also, take a sufficient amount of liquid (water, hot drinks), vitamins in the form of fresh fruits and vegetables, fish. On the other hand, the most important thing is to avoid meetings, wear a mask both indoors and outdoors if there is a crowd. Do not forget the importance of walking, at least two kilometers a day in areas without crowds, with a mask on your face – says Dr. Vukicevic.
