The potential risk of increasing the number of coronavirus patients is the glory period that begins … Currently, the youngest patient in the Infectious Diseases Clinic is 24 years old, and the oldest 93, and we do not yet know the possible consequences of SARS-CoV-2 infection in the liver, central nervous system. reproductive system and organs, and I call on everyone to strictly respect preventive measures so that we do not have the situation of a few months ago.
This is what doc. Dr Ivana Milošević, Deputy Director of the Clinic for Infectious and Tropical Diseases in Belgrade of the Serbian Clinical Center, who reveals what consequences kovid leaves on the body even after recovery, why Belgrade is the largest hotbed of the virus, but also that they had cases of coronary infection.
Epidemiologists have been warning for weeks that we are entering, or have already entered, the third wave of the crown in Serbia. What does this mean exactly and what can we expect in the coming days?
– Typical weather conditions for this part of the year mean a shorter stay outdoors and a longer stay indoors. It is the stay indoors, especially of a greater number of people, which affects the transmission of the coronavirus infection. In the coming days we must, without exception, follow the recommendations that have been in force for months if we do not want to have the situation of a few months ago again. This must be the common goal of all the people of our country.
Do you expect a new jump in the number of patients and what could be the cause?
– Any relaxation in terms of non-compliance with the recommended measures inevitably leads to a jump in the number of patients. The migration of people is not as important now as during the holidays, and the returnees from abroad at the border crossing are instructed on the supervision procedure and the self-assessment test. The potential risk of increasing the number of patients is the period of glory that begins. Glory is part of the identity of our people, a solemn, family party, especially spiritual in honor of the saint, and in that sense it should be celebrated.
How many corona patients are currently cared for by the Infectious Diseases Clinic in Belgrade and how many are on respirators? Is your clinical picture different from the previous “waves” and what is it like?
– The Infectious Diseases Clinic has 45 patients, five of whom are connected to a respirator. It is mostly an active labor force. We did not notice a difference in the clinical picture of current patients compared to those we had previously treated. The severity of the clinical picture varies and patients who need oxygen require special attention, because a part of them will be on artificial ventilation. They are mostly older people with associated diseases: hypertension, diabetes, chronic lung diseases …
It is increasingly being said that large numbers of kovid patients end up on dialysis even after recovery, often developing thrombosis … Have you come across this in your long clinical experience? What are the most common side effects and which organs does kovid 19 have the greatest consequences for?
– Severe forms of kovida 19 can lead to pulmonary fibrosis, ie. scar tissue in the lungs that disrupts their function. We have long understood that this respiratory infection also has consequences in other organs. Kidney failure occurs in the most severe forms of corona in breathing patients who have dysfunction of other organs. In that case, we are talking about patients with a poor prognosis … It is clear that the so-called acute renal failure means a probable result of the poor disease. Previous chronic kidney disease, similar to other chronic diseases, is also considered a poor prognostic factor. It is known that kovid can also cause cardiovascular manifestations: myocarditis, pericarditis, heart failure. Since the beginning of the pandemic, we have been aware of the tendency of these patients to develop thrombosis. Therefore, anticoagulant therapy is an integral part of the treatment protocol. We do not yet know the possible consequences of SARS-CoV-2 infection on the liver, central nervous system and reproductive organs. To obtain definitive conclusions and attitudes, it is necessary to follow the patient for a longer period of time.
Do you know where your current patients were mainly infected? In your opinion, should the current restrictive measures be strengthened? Are there places, such as cafes, public transport, or large buses, that pose the highest risk of infection?
– Our patients generally know how and when they were infected and most of the time they provide information about contact with a sick person. Any stay indoors without wearing a mask, keeping your distance, and proper hand hygiene is an absolute risk of SARS-CoV-2 infection. It is inadmissible to remain in any enclosed space without a mask. Whether the measures will be toughened is a question for epidemiologists, and for us physicians, the most important thing is undoubtedly to have as few patients as possible.
Have you had any cases of reinfection or reinfection with the crown in the Clinic and, if so, how many?
– The fact is that there is no guarantee even for a person who has already had a covid, and that even those who have contracted the disease can be re-infected. We have had such patients in our practice. It is true that their number is small.
Why has Belgrade been convincingly the biggest focus of coronavirus in Serbia for months now, and what can we do to improve the situation?
– Belgrade is a city of two million with all the advantages it brings, and in this situation, unfortunately, the disadvantages of living in the metropolis. The daily need to go from one end of the city to the other, the great daily migration, the fact that it is the largest university city and the administrative center of our country, implies through contacts and risks that are not typical of an environment smaller. However, we have learned that we can control the situation on our own with very simple measures such as the use of masks and hand hygiene, and once again I ask your readers to do so.
When do you think the first kovid vaccine will appear in Serbia and will it be effective enough? Will the seasonal flu vaccination “eliminate” at least one enemy and help in the fight against the corona?
– The production process of the new vaccine is very complex. We are witnessing a positive international “race” to get the vaccine as soon as possible. I want it to happen as soon as possible, but it is difficult to predict when it will happen … Until then, I call on our citizens to get vaccinated against the flu. The vaccine is available and now is the right time for it, because vaccination is the most important and safest way to prevent an infectious disease, and any additional risk of disease must be minimized in the non-pandemic period, and now especially.
