IN THE THIRD WAVE, HEALTHY END IN THE RESPIRATOR! Dr. Janković: Now, the most threatened people are people aged 50 to 70

Patients between 50 and 70 years old, who did not have any associated disease, are on mechanical ventilation. The corona continues as a lung disease, and attacks all organs, says Radmilo Janković, director of the Anesthesia and Intensive Care Clinic at the Nis Clinical Center.

The number of covid patients requiring intensive treatment and oxygen support is increasing, and 13 of them are already on a ventilator at the Nis Clinical Center.

– While at the peak of the first wave, in March and April, we had mostly patients aged 50 to 70 years, even older, and many of them with chronic diseases, and in the second, in July, many young, now we have the majority of patients aged 50 to 70 years who do not present significant comorbidities, that is, associated diseases. The youngest patient on mechanical ventilation is from Nis and is 46 years old, Janković said.

It says they ended up on a respirator because of the corona, and if that’s a characteristic of this third wave / peak.

– They were mostly healthy before the crown. These are middle-aged to older people, 50 to 70 years old, and most of them are from the extreme south, from Vranje, Presevo and Bujanovac. Up to 90 percent of them, both on the ventilator and in the intensive care unit, have this age structure. I don’t think they were directly infected, but it would be more likely that they were younger carriers and that these people became more seriously ill – says Janković.

According to him, the main problem is currently young people.

– They spread the virus. And its movement, in the afternoon, at night. I’m not talking about schools, not even faculties, but what the young virus brings after the evening meetings – Janković points out, adding that there is no indication that the patient’s clinical picture is different in the intensive compared to the two previous peaks.

It is a classic image of a systemic disease that begins as a lung disease and takes patients to the doctor precisely because of shortness of breath. But then it develops as a systemic disease that attacks the kidneys, liver, and other vital organs. Finally, death never occurs only from lung failure, but also from the systemic nature of the disease.

Does the crown still destroy the whole organism?

Janković says that two ventilator patients are on dialysis at KC Niš, which means their kidneys have failed.

As he himself says, he is surprised that the number of infected people, not only in Nis but in all of southeastern Serbia, is not as great as in March and April, and in July and August.

– In March and April we had most of the patients from Nis, and in July and August from other places in southern Serbia, from Kosovsko Pomoravlje, Vranje, Presevo, Bujanovac … I think it is only late and that this wave will surely us reach. At the moment, we are more than stable. We have 90 covid patients, our occupation is a little more than 30 percent because there are currently 240 places ready. Intensive care is a bit more crowded, 35-40 percent. If there is a lot of rush and a big hit, we can release five times more capacity. The problem with us is that we have to be a red and green hospital and that there is no other great tertiary institution that is a green hospital. So, we receive severe covid cases, as well as all the others, that is, those with multiple trauma, which are for difficult operations … Our problem is that in the capacities we have, which are 1,530 beds, we place both “green” and “red”, both mothers and children – explains Janković.

According to him, KC Nis managed to organize itself as a green and red hospital.

– Since yesterday, we have switched back to another operating mode: emergency only. But that means cancer surgery works – cancers don’t wait for surgery, radiation, consultations, or chemotherapy. Dialysis goes smoothly. We have separate “green” and “red” radiology, so the magnet, the scanner, and everything else are divided. However, 20 anesthesiologists in intensive care have not been in an operative program since yesterday – Janković said, adding that those operations that can wait, such as gallbladder surgery, hernia …

Asked when the peak of this third wave is expected, he says that at the moment it does not seem that we will reach March and April.

– But, if the number increases in all parts of Serbia, it will also merge here. Based on these numbers, we certainly won’t be full for another week – Janković added.

There are currently Novi Pazar patients at KC Nis.

– They called us and asked to receive them, they want them to treat them with us because, as they say, we are very good. Our capacities have not been filled and we have met with satisfaction – said Janković.

Woman gave birth with pneumonia, negative test

– We had a pregnant woman with pneumonia, who was negative twice. She gave birth last week, the delivery went well and the baby and the mother are still with us, says Janković. According to him, about 90 percent of them are positive in the test and that 10 percent are negative. Among Kovid’s patients are six children who are mildly ill and have no babies. At KC Nis, there are also married couples staying in the same rooms and we still don’t have patients from the same family on the ventilator.
