Recognized as one of the largest public health systems in the world, the Unified Health System (SUS) turns 30 this Saturday (19). In the assessment of the Minister of Health, Eduardo Pazuello, the SUS became fundamental. “There is no other way out for our country in health matters, other than the strong and efficient Unified Health System,” he said. The next 20 years, he added, is already being prepared by the folder “We are setting up structural actions with strategic projects in all areas, such as Digital Health, Genome Project, among others, which are being finalized,” said the minister.
In the ministry’s evaluation, with the new coronavirus (covid-19) pandemic, it is possible to verify the strength and importance of SUS, which serves about 70% of the population. Under the management and union of the three entities – federal government, states and municipalities – the folder says that assistance to patients infected with covid-19 and the care of those who need specialized treatments could be guaranteed.
The president of the Brazilian Medical Association (AMB), César Eduardo Fernandes, makes reservations to face the current pandemic. For him, the SUS response was “reasonable to good.” The doctor praised the re-equipment of many hospitals and the formation of health teams in recent months, but raised questions about whether these advances will be maintained or will return to the pre-pandemic stage.
Fernandes added that the response could have been more efficient if primary care had not lost investments in recent years. “In this period of pandemic, professionals would be more prepared to provide the first care and a correct filtering of these cases, there would be no need for this massive trip to hospital services,” he assessed.
Challenges
Among the great challenges of SUS, in the evaluation of the Ministry of Health, are the offer of services and the financial part. Amid ever-increasing demand, portfolio experts admit that the service must be efficient to meet all these demands and needs in a timely manner. They also believe that resources must also be distributed to achieve the best possible result.
A frequent target of misdirection by scammers, liability for public resources is also a challenge. “We need to have efficiency, transparency and responsibility with public resources, because we are not talking about money, we are talking about people’s health”, defends the Minister of Health, Eduardo Pazuello.

Regarding the efficiency of the SUS, the president of the College of Physicians assessed that in 30 years of existence the public health system has managed to offer excellent services in some areas, but still suffers from insecurity. In the evaluation of the doctor, a greater investment is needed in the careers of health professionals and in primary care.
“We cannot keep these islands of excellence in large centers, large capitals and more developed regions. We have to internalize the SUS ”, defends Fernandes, speaking of the challenges that public health still has to face in the country.
Basic health units and family doctors should also be a focus of this expansion, said César Fernandes. “What needs to be strengthened throughout Brazil are the basic health units. It is there that the patient arrives, that the first appointment is made, the diagnosis is made and the treatment begins ”, he stressed.
The doctor emphasizes the importance of also having investments in the public career of the class. “Our question is not a lack of doctors, it is to build possibilities for the young, well-trained doctor to have the attraction of going to the small centers and the most distant cities. We have to create the figure of the State doctor, just as he has a career in the Judiciary ”, he exemplified.
