A helicopter engine failure was the cause of accident aerial that caused the journalist’s death Ricardo Boechat, according to the final report prepared by the Center for Research and Prevention of Aeronautical Accidents (Cenipa), gives Brazilian Air Force (FAB). According to the document, there was also a “failed landing attempt” and the aircraft was “destroyed” after colliding with a truck.
The report notes that the accident occurred around 2:05 pm, just 20 minutes after the flight took off from the helipad at the Royal Palm Plaza, in Campinas, bound for São Paulo. In the images of the security system, it can be seen that the pilot tried to land in the grass area between the two upper strips of the Mário Covas Bypass. When it could not maneuver, it guided the aircraft under the viaducts, when it collided with a truck in flight.
The pilot Ronaldo Quattrucci, -6 years old, who also died in the accident, did not have the mandatory certificate of a training program approved by National Civil Aviation Agency (Anac). The report also indicates that the transport used was not regular in the form of air taxi and that the company RQ Serviços Aéreos Especializados Ltda was not certified or authorized to provide the service.
Another point cited by the FAB is the date of the last general overhaul of one of the compressors, which dates back to 1988. During the special technical inspection, it was found that the part had been replaced by a newer one during an evaluation carried out in 2017 by Anac, and reinstalled on the aircraft less than three months later, even with expired authorization.
In conclusion, the document indicates six main factors that contributed to the accident: the attitude of the pilot when not observing the necessary maintenance actions; the organizational culture of the companies involved in the rental of aircraft; “flight indiscipline”, whose operator did not have authorization or certification; the “piloting test”, considering that the helicopter’s engine was suitable for take-off; aircraft maintenance; and organizational and decision-making processes.
“There was inefficiency, both on the part of the operator and the maintenance organization, in the monitoring and execution of maintenance processes,” the document concludes. “The operator knew that installing the compressor module without overhaul, as well as exceeding the oil change intervals, was contrary to the maintenance schedule for the aircraft engine, so it was ‘not flyable’.”
