“I think a lot of people who say that kind of thing have never treated a covid-19 patient,” says an intensive care physician who has treated 1,000 corona patients.

American Kenneth Remy was saddened and resigned after he once again had to pick up the phone to convey a death message. Therefore, he ended up making a video that has received enormous attention on social and traditional media.

Remy works as an intensive care physician at the Missouri Baptist Medical Center in St. Louis, Missouri. The hospital has been badly affected by the corona pandemic. He has treated more than 1,000 patients with coronary heart disease and has performed the intubation procedure on more than 100 of them.

Intubation is a procedure in which the patient has a plastic tube inserted into the windpipe with the intention of supporting or taking over the breathability of the machines.

Two weeks ago, he made a video simulating intubation in a patient with Covid-19. He produced a long scope and a plastic tube and held it up to the camera.

“I hope the last moment of your life doesn’t look like this,” he says in the video.

He uploaded the video to Facebook and Twitter, where it has received hundreds of thousands of views. Additionally, the video has been shown on various national and international television channels, including CNN and the BBC.

– Why did you make this video, Remy?

– In our intensive care unit, we have had a fairly significant increase in covid-19 disease. We have seen many critically ill patients. Many people know that if you become infected with the coronavirus in the local community, the probability of survival is greater than 97-99 percent. But as soon as you enter the hospital with a severe course of illness, your chances of survival will decrease. And as he enters the intensive care unit, the chance of survival decreases even further, Remy tells Nettavisen.

– Last week I treated several patients in the intensive care unit. I had to call several spouses, about ten. Calling a loved one in the middle of the night to tell them that their loved one has died is not an easy task. One of the people told me that they had been in a meeting of many people without masks and without social distancing. It was very difficult for me to deal with it. I looked at a very experienced nurse and could see the defeat in her eyes. So I grabbed the laryngoscope and endotracheal tube (laminated editor’s note) and made this video, he says.

– I didn’t realize that it should be seen by more than a million people in many different news. At least the idea was to send a message to people that infection in the community can be prevented. If unfortunately you are faced with a severe course of illness and put on a breathing tube, we know that only in that cohort will between 30 and 50 percent die.

– This is real. This is not fear. It is not to scare. Unfortunately, that is real. I wanted to show the public that this is a serious disease. And since many visitors have been denied admission to hospitals for fear of transmitting infections, this is the only way I can show what it looks like for a small percentage of the population who are so unlucky that they end up in this situation.

Painful death?

– Has processed more than 1000 covid-19patients and more than 100 patients were intubated. How many of them have died from covid-19?

– Of the more than 1000 patients I have treated, unfortunately 35 to 45 percent of those with intubation have died.

– Is dying from covid-19 painful?

– End up in a situation where you can’t breathe, and you breathe 30-40 times a minute, and feeling like you can never supply oxygen to the glass, is clearly a problem for people. If it is painful as such, we at least do our best to make it comfortable for those suffering from pain.

– This applies to those with breathing tubes as well as those who do not, but are receiving non-invasive ventilation treatment (mask treatment, editor’s note). We try to make sure they are comfortable. I would have liked that for my part. So the answer to your question is that my patients with breathing tubes are squeezing my hand, but they haven’t expressed that they are in a lot of pain. Thank God. And we will do everything in our power to prevent that from happening.

– In both Norway and the US, there are some cynical critics who claim that the majority of COVID-19 patients who die are very old, that they would die of another disease in the near future, and that the majority die. with covid-19 and not off COVID-19. How do you react to such an argument?

– I think that many people who say that kind of thing have never treated a patient with covid-19. So I think that belief, so to speak, is not necessarily based on factual information. Many of the patients I have treated who have sadly passed away are people in their 40s, 50s and 60s. Many of these people do not have significant comorbidity (multiple diseases at the same time, editor’s note) beyond morbid obesity, diabetes, or high blood pressure, Remy says.

– It is absolutely true that people in their 70s, 80s and 90s who have other comorbidities, and who are actually older, have a higher risk of potentially dying. However, we have seen a significant increase among people aged 40 to 59 who actually die from covid disease. They die of refractory hypoxemia (low oxygen content in the blood, unaffected by treatment). A proportion of that group have T-cell fatigue, which means the inability to have a good response from the host, which cannot fight the virus itself. Also, they are prone to secondary infection (infection in addition to the underlying disease). Then they have a condition similar to sepsis (inflammatory condition in the blood), he says.

– Many of these patients also have what we call microthombosis, which involves clots in specific blood vessels, which can be part of the pulmonary circulation. Either you have a stroke or you suffer from cardiac arrhythmia (disturbance of the normal rhythm of the heart). All this is caused by the covid infection. It is not due to another disease, he emphasizes.

– I think it is difficult for people who have not treated patients to understand this. I think it is easy to assume that the disease is not as real as it really is. On the other hand, there are many, many morgues around the world that are full of patients. And as we move toward social distancing, mask use, and hand washing, we’ve been fortunate to have a lower incidence of many common respiratory viruses that we would otherwise see at this time of year, especially in winter, such as human metapneumovirus, influenza. , rhinovirus and parainfluenza.

– The incidence of these diseases is not as high as a year ago. So our strategy really helps keep people from ending up in hospitals. These people are not dying from the diseases they would have died from last year. They die of covid. That is what they are dying of. To anyone who believes otherwise, I encourage you to speak with healthcare professionals who have treated patients. I can promise you that I have no interest in making up data or conveying anything other than the truth. My hope is that we will overcome this pandemic so that we can restore normalcy to our society, he says.

Light at the end of the tunnel

– What is your message to people who are tired and weary of infection control measures like bandages, social distancing and partial closure?

– Let me be very clear. I am not in favor of closing our entire society. I don’t want restrictions imposed that shut down the economy or small businesses. In fact, I mean the opposite. The only way to keep things open is to follow preventive measures that reduce the transmission of the virus, which has been confirmed in many studies. And that includes wearing a mask, washing your hands regularly, and making sure you keep a safe distance from those who are not part of your normal social bubble. And avoid large gatherings, he says.

– If you follow those principles, you will reduce the transmission of infections and we can continue to keep things open, at least until further notice, and allow children to go to school, allow our small businesses to remain open. What I want to say to people is: Listen, I know you’ve been doing this for 11 months. I understand that everyone is very tired.

– There is a light at the end of the tunnel. We have excellent vaccine candidates that will help us achieve herd immunity, and hopefully we will end this pandemic in the coming months. I urge you, please, to follow these precautions, so that small businesses can stay open, so that children can go to school, and so that people do not die. I can promise you it works, says Remy.

1.5 million deaths

Remy is also the head of a covid laboratory at the University of Washington Medical Center, which is testing new treatments for the covid-19 disease.
