[바이오타임즈] Two allergic reactions were reported on the first day in the UK, the first in the world to be vaccinated against the new coronavirus infection (Corona 19) developed by Pfizer-Bioentech.

According to the BBC on the 9th (local time), two employees of the British National Health Service (NHS) had an allergic reaction immediately after vaccination. The allergic reactions they suffered are ‘Anaphylactic Reaction’ which causes skin rash and drop in blood pressure, which is weaker than Anaphylactic Shock. The condition is said to be in good condition after receiving treatment.

Consequently, the British Medicines and Health Administration (MHRA) is investigating whether allergic reactions are related to vaccines and urged citizens with a history of allergies to avoid vaccination. Additionally, the MHRA mandated that corona vaccination not be administered in facilities without CPR equipment.

Pfizer and Bioentech, a vaccine developer, are cooperating with UK authorities to determine the cause of the allergic reaction, but basically they say that no serious safety issues were seen in the phase 3 clinical trial that involved more than 42,000 people.

These allergic reactions have already been reported to the FDA, but reactions such as headache or fatigue that occurred in less than 4% of cases were not treated as major side effects because their frequency was very low. According to a report published by the FDA, allergic reactions to the COVID-19 vaccine are approximately one in every 1,000 people.

As such, allergic reactions resulting from vaccination with Pfizer are the common reactions to the vaccine, but concerns about side effects continue to rise.

According to foreign media reports on the 9th (local time), as a result of a Corona 19 vaccine clinical trial conducted by Pfizer in the United States, 4 of the 21,720 people who received the vaccine showed symptoms of facial paralysis. Since this case of side effects was not disclosed in advance, some were concerned that there was an issue with the safety of the Pfizer vaccine. However, the FDA’s position that the proportion of all clinical participants who complain of symptoms of facial paralysis is lower than the usual prevalence of facial paralysis.

Among these, the Canadian government approved the Pfizer COVID-19 vaccine for the third time in the world on the 9th (local time), and the Bahrain government also approved the use of the Pfizer vaccine on the 4th.

Health Canada said it is recommending that people who have had past side effects from Pfizer’s corona vaccine not get the vaccine.

◇ Can the Pfizer vaccine be said to be safe from anaphylaxis?

Vaccine allergy is known to be one of the most common side effects. The proper immune response through vaccination produces antibodies and makes the body remember the virus, but if this immune response is excessive, an allergic reaction occurs.

Anaphylactic shock is a serious adverse reaction that can occur after vaccination. Anaphylaxis is an overreaction in the body to certain substances and symptoms such as rash, vomiting, shortness of breath, chest pain, rapid heartbeat, and low blood pressure appear immediately. If you don’t take quick first aid measures, life is dangerous.

The substances that cause anaphylaxis are mainly penicillin antibiotics, antipyretic pain relievers, vaccines, foods including eggs, peanuts, shellfish, and fruits, and can also occur from bee stings or insect stings.

As a result of an analysis of patients who recently visited 16 hospitals with anaphylaxis, one in five patients was found to be a serious and life-threatening patient.

A research team from Ajou University Hospital, Professors Soo-young Lee and Kyung-wook Jung (Department of Pediatrics and Adolescents) and Professor Young-Min Ye (Department of Allergy) analyzed data from 558 anaphylaxis patients enrolled in 16 hospitals from November 2016 to December 2018.

According to the study results, 131 of 558 subjects (23.5%) were patients with severe anaphylaxis and experienced one or more of the severe neurological symptoms such as hypoxia, hypotension, and loss of consciousness. In particular, severe anaphylaxis was more common with age and was caused by poison from insects, drugs, food, etc. In particular, insect venom was found to be the most dangerous.

The age range of 558 subjects ranged from 2 months to 84 years, of which 60% were children under 18 years of age. Anaphylaxis was caused by food (84.8%) in children and adolescents, drugs (58.3%) and food (28.3%) in adults, and other causes were insect poison, exercise, and unknown cause.

In particular, most infants and young children experienced food-induced anaphylaxis, and as adolescents age, the rate of food-induced anaphylaxis decreased and drug-induced anaphylaxis gradually increased. Furthermore, many anaphylaxis caused by insect venom occurred in old age.

The main causes of anaphylaxis were antipyretic analgesics and antibiotics in children and adolescents, followed by antibiotics, antipyretic analgesics, and H2 receptor antagonists (inhibitors of gastric acid secretion) in adults.

When anaphylaxis occurs, the most important treatment option is to use epinephrine. If you have portable epinephrine, you need to inject yourself into your thigh first and then go to the hospital right away. In addition to epinephrine, antihistamines, steroids, and blood pressure stimulants are also given to relieve symptoms.

Anaphylaxis is diagnosed primarily based on medical history and symptoms, when a person who was relatively intact rapidly develops multiple organ symptoms (skin, respiratory, cardiovascular, and digestive) after exposure to certain substances, it is diagnosed as anaphylaxis.

Although the probability of anaphylaxis after vaccination is extremely small, there are voices that the latest technology, such as Pfizer’s Corona 19 vaccine, must be ensured.

On the 10th, Professor Kimoran National Cancer Center, who is in charge of the 19th Crown Countermeasures Committee of the Korean Society for Preventive Medicine, appeared on TBS ‘Kim Eo-Jun’s News Factory’, and Pfizer’s vaccine uses the latest technology applied ‘messenger ribonucleic acid’ (mRNA, messenger RNA). He said that it should be noted that it is a hexane vaccine.

Professor Moran Kim said of Pfizer’s Corona 19 vaccine: “Because we have never vaccinated a public on a large scale, it is the first time it has been applied in the field. “New news will continue to emerge during vaccination in the UK, and it is helpful because we can monitor the situation and prepare for it.”

In addition, he noted that the Pfizer vaccine should be stored and distributed at -70 degrees Celsius, △ It must have a dissolution process, △ It must be used within 6 hours after mixing with saline after thawing, and many difficulties are expected to a safe vaccination. did.

In particular, I did not forget the warning about side effects. Professor Ki said: “If there are a few thousand people like now, but there are two anaphylaxis, there are too many things to consider, such as having to prepare medical personnel and drugs in preparation for a dangerous situation.” I mentioned that it is great.
