According to the instructions sent to the vaccination centers and prepared by the Ministry of Health and the National Vaccination Committee, citizens who will be vaccinated against Covid-19 must give their consent orally, which will ensure that they have been informed. completely for the process, but also for the possible side effects. In the event that a citizen cannot give oral consent to the vaccine, the document must be completed by a family member, caregiver or legal assistant.

How to make an appointment

No electronic signature will be required, identification will be made by the state.
