Scenes such as those of the last two days in various commercial sectors of Bogotá, in which people abound and personal protection measures are scarce – social distance and correct use of the mask – motivated the union of intensivists and academics of the medicine raised a voice of alert.
The Colombian Association of Critical Medicine and Intensive Care (Amci) raised considerations and reflections on the so-called new reality after the end of the general quarantine last Monday.
“Despite all that has been gained in terms of response capabilities and what the pandemic has represented in loss of human life, Colombia faces the great debate about whether the protection of health and life is compatible with the reactivation of the economy. A dilemma that arises under the premise that the pandemic is not under control, that mathematical models are riddled with biases, and that epidemiological information may underestimate reality”, They warn from the beginning.

And they point out, specifically, that the interaction of infected, symptomatic, asymptomatic people with vulnerable “In a scenario of lack of awareness, individual and collective, poses a high risk of contagion”.
The specialists mention several situations that concern them: that the circulation of the virus and its spread in the community persists; that it is unknown how many of the new cases include patients as probable cases; the ‘boomerang effect’ that has increased the use of ICUs by other neglected pathologies in quarantine; and the “shortage” of human talent trained for this care; among other.

“The economic reactivation and a strategy of selective isolation they bring with them a high risk of new waves or spikes of covid-19 with a high number of infections, affecting a larger population at high risk, with a greater number of fatal outcomes “, they indicate.

Intensivists recommend wearing a strict, real-time, public monitoring of the pandemic to parameters such as contagion rate, mortality, ICU occupation and daily cases in remission. But also generate hospital red alert if ICU occupancy reaches 70%. And, on the other hand, do door-to-door community pedagogy and active search for cases and isolation.
The Amci also considers actions to protect human talent in health and the guarantees of their physical and emotional integrity and work well-being as key.

“It is possible to achieve a certain degree of balance and harmony between a social and economic reactivation strategy with the ability to adequately care for critical patients, to the extent that this reactivation is done gradually and progressively and risk mitigation actions are implemented in a rigorous and responsible manner and under strict supervision ”, they conclude.

From the academy
In the same sense, Fernando Galván, vice dean of the Faculty of Medicine of the National University, pointed out that if businesses and entrepreneurs do not comply with the established measures or respect shifts and schedules, The projections estimate for October and November more than 9,000 cases of general hospitalization and almost 3,000 cases of ICU care in Bogotá, “A scenario that would exceed the installed capacity of the
city”.

In contrast, in a scenario in which people comply with self-care measures according to the new normal and the economic and business sectors rigorously assume their shifts and schedules, by the end of October there could be about 6,000 cases of general hospitalization and 1,700 requiring ICU care, Galván scores.
“People must understand that the new coronavirus did not disappear, it is still present, only that social and economic conditions make it necessary to reopen different activities, but what is seen is that in some areas not all the measures, ”says Galván.

The academic affirms that the message for citizens should not be limited to self-care. “If it is sent from individualism it can cause that due to the recklessness of some many people end up infected”, he concludes.
