We sign a special document before getting vaccinated against COVID-19

The Ministry of Health has published the declaration of informed consent for vaccination against COVID-19, which will be signed by anyone who wishes to be vaccinated. In it, the names, PIN, DNI and address of the person to be vaccinated are entered. Consent can also be given by a parent, guardian, or administrator.

The declaration establishes that the undersigned knows the benefits of the vaccine against COVID-19, which has been chosen and its commercial name, agrees that it should be administered, and that in case of adverse reactions after vaccination it should be communicated its appearance at any time to medical professionals or the Executive Agency for Medicines. The document includes the date, the person vaccinated and the doctor who marks the dose. The Minister of Health has already issued an order with which vaccination will begin in our country on December 27.
