Regarding the students – The coupon for the student’s health and immunization status from the start date of the school year, the so-called prophylactic health card or personal card, must be delivered to the school authorities.

This can be done by both the father and the GP, ex officio.

Given the situation of the epidemic and to avoid congestion in front of the GP’s offices, the coupon can be delivered up to a month after the start of the school year, and not as before, within 7 days.

When mandatory immunization or re-immunization from the National Immunization Schedule is carried out later, additional data may be provided to the school.

Children who have recently arrived and are absent for a long time from nurseries and kindergartens should investigate

With regard to children attending nursery schools and kindergartens:

The following documents are required for children newly admitted to kindergartens:

– health prophylaxis card, completed by the personal doctor, and information on the child’s vaccination status;

– negative result of a test for pathogenic intestinal bacteria and intestinal parasites, carried out no earlier than 15 days before entering kindergarten;

– blood and urine tests carried out within a week before the child enters kindergarten;

– Medical note from the family doctor due to lack of contact with a contagious patient.

The following documents are required for children newly admitted to daycare:

– health prophylaxis card, completed by the personal doctor, and information on the child’s vaccination status;

– negative result of a test for pathogenic bacteria and intestinal parasites, carried out no earlier than 15 days before entering the nursery;

– blood and urine tests carried out within a month before entering the nursery;

– a test with a negative Wasserman reaction for one of the parents, carried out within 6 months before entering the nursery;

– Medical note due to lack of contact with a contagious patient.
