The government commission to investigate the AN-26 disaster cited the reasons for the accident that resulted in the deaths of the cadets. Various reasons were given.
This was stated by the head of the commission, the deputy prime minister and minister of Strategic Industries, Oleg Urusky, at a government meeting.
According to him, the main causes of the accident were lack of effective control over the organization of flights by officials of the Kharkiv National Air Force University, named after Kozhedub, systemic violations flight rules in college.
Also the reasons are insufficient professional training of the crew to special situations, take off by transporter (made 6 flights in a row) engine control system failure, violation of the scheduled flight schedule.
Thus, Urusky named 6 dangerous factors that led to the plane crash:
- Left Engine Control System PRT-24 Failure,
- the crew failed to align the thrust of the left and right engine,
- the crew extended the flaps to 38 degrees and took the wrong descent path,
- premature transfer of the aircraft to descent and false descent formation,
- artificial creation of asymmetry of the thrust of the main installation,
- no slip elimination.
In addition, the head of the commission pointed out that the crew was tired and carried out flights on the conveyor belt without rest and checking technical operability. The planned flight table was also violated.
“There were people on board who were not included in the scheduled flight schedule,” the deputy prime minister said.
Earlier, in a comment to RBK-Ukraine, Oleg Urusky said that the causes of the plane crash were established. The human factor was superimposed on technical failures.
The investigation considered four versions of the accident of the military plane AN-26 in Chuguev.
