Covid infection is increasing rapidly in Stockholm nursing homes

The latest report from the region shows that 48 nursing homes, which is 15 percent of all nursing homes in the region, now have one or more cases of Covid-19. Between November 6 and 9, 4,762 new cases of covid-19 were found, of which 103 were sampled in special accommodation.

– It is incredibly worrying and serious that we have such a large spread of the infection now that it is also entering Säbon. The infection is accelerating rapidly, we have also had a sampling break, so there may be even more, says Maria Rotzén Östlund, a physician in infection control functions in the Stockholm region.

The region is now drafting new guidelines to track infections faster and more widely in nursing homes. Evaluation of staff and accommodation is also discussed, as many infected people have no symptoms.

– Firstly, we change the guidelines so that infection tracking is better, then we have to discuss detection after that, unfortunately we have to take one thing at a time, says Maria Rotzén Östlund.

Nine municipalities in the county have decided to totally or partially discourage visiting nursing homes and nursing homes. This applies to Botkyrka, Danderyd, Lidingö, Norrtälje, Nynäshamn, Täby, Upplands-Bro, Upplands Väsby and Solna.

– In Solna, we have introduced a strong deterrence of visits until further notice due to the increased spread of infection in the county. We cannot ban anyone, but we hope it will take effect. We have an increase in occasional cases of infection in nursing homes, less than five at the moment, says Rasmus Sandsten, media manager at Solna.

The city of Stockholm does not advise against it of visits, but the Senior Citizen Council has given business managers the right to close nursing homes that have contracted the infection if necessary.

– I would rather work with local restrictions than put one stop on visiting all the nursing homes in Stockholm. I do not want to advise in general, although I do not think it is appropriate to take the whole family there. You may not accept grandchildren who are in a preschool setting, but narrow the circle, says Erik Slottner (KD).

In the city of Stockholm, the number of infected people in homes doubled last week. At that time, there were eleven covids infected in nursing homes and care and seven who were cared for in hospitals or other care. In the home care service, 27 older people were found to be infected.

– We have contracted contagion in some homes and we have some affected elderly people. There is not a great spread of the infection yet and we hope it will continue to be so, but it is very unfortunate, says Erik Slottner.

He doesn’t think the infection is spreading in the same way as last spring, when 46 percent of those who died in Covid-19 lived in nursing homes.

– We are very prepared now than when it was at its worst this spring. We are better equipped with protective gear, staff have better knowledge and routines, and the region tests and tracks infections to a different degree today, he says.

Since November 6, another 11 people in the Stockholm region have died from covid-19. 393 patients are cared for in the hospital, of which 90 are cared for in a geriatric clinic and 36 in intensive care.

I Stockholm Region Special guidelines developed to care for patients at the appropriate level of care have been reviewed, including by DN. The use of a frailty scale to determine the priority of healthcare for people in nursing homes or LSS homes has been tempered by comments from researchers about the risks of discrimination.

The Swedish Health and Care Inspectorate, Ivo, has found that the assessment of the care needs of older people is more general than individual.

At the same time, emergency hospital care is still recommended only when emergency care is needed for the elderly in special housing, for example in the case of emergency surgery, according to the current guideline. The purpose is to secure the resources of emergency hospitals for the most seriously ill in covid-19.

– These are the same guidelines that apply now. But it is as before the individual evaluations that are applied. We do everything we can to ensure that older people receive the most advanced care where they are, because there are risks when transferring patients, says Director of Health and Medical Care Björn Eriksson.

About the elderly in nursing homes they need to be transferred, for example, to receive oxygen treatment, it is mainly to the geriatric ward where the places are expanded, finally to the emergency hospital. One of the objectives is the medical evaluation that will be carried out in the accommodation. As reported by DN, the physical doctor’s visits to special accommodations decreased during the most intense months of the pandemic. Björn Eriksson does not provide any information on the doctor’s assessment that should be performed by an on-site doctor.

– It is the person who makes the assessment who decides how it goes. It should always be easy to obtain a medical resource for an individual evaluation.

How is that ensured?

– We try to inform operations so they know that there are resources available. This time, the specialty lodgings are much better prepared and so are we.
