While medical institutions have high hopes for this system, hospital representatives who already have experience in using it do not hide that not everything works as if it were buttered and there really is room for improvement. Therefore, registering in the system poses problems for both patients and doctors. Family physicians are especially reluctant to do so as they constantly face e-health disruptions.
Not all institutions are in a rush to connect
There are almost 920 treatment facilities in the country that could use IPR, but today contracts have been signed with only 409, and only 204 of them have already joined and specialist visit times are increasing.
According to Diana Vilytė, Director of Service Management at the Registry Center, 112 of them work through integration interfaces using their own systems, 92 use the IPR portal. Of the participants, 43 are private health centers.
“Some institutions are not yet in a hurry to connect, because they are used to using sergu.lt and funds are needed to install the infrastructure. There is also a certain brake on the obligation to announce all possible visits, so such a full opening may not be acceptable to everyone. Of course, the pandemic also slows down the processes, ”he mentioned in the Seimas Health Affairs Committee as a possible reason.
Although in 2020 sergu.lt was supposed to be the last year of operation, but the Ministry of Health (SAM) has yet to continue its activities due to the failure of some institutions in data transfer. However, it is already possible to use sergu.lt from April and it will no longer be possible to declare schedules to the institutions there.
A challenge for elderly patients
Still, despite the expected benefits of the new system, a large proportion of patients still register simply by calling the treatment center.
It is noted that these changes may present the greatest difficulties for elderly patients who are not used to connecting to the system through the e-government gateway.
“It just came to our attention then. And after that, you still have to request a personal code, which is not what everyone wants to say either. Also, they are still trying to connect via sergu.lt, where they don’t show the hours, though actually they are, ”said Eglė Ramanauskienė, representative of the Kaunas clinics.
The problem of such duplication has been raised in the past, but SAM explained that this is an unavoidable problem during systems transition and relocation.
SAM has already informed the institutions that have already joined to fix the schedules and transfer them to IPR so that sergu.lt has nothing left. In other cases, when the institution has already done so, the connection is automatically routed to the new system, ”explained Vilma Telyčėnienė, SAM representative.
No more occupying multiple seats
Jolita Jakutienė, deputy director of Ambulatory Medicine and Nursing at VUL Santara Clinics, emphasized that it is good that a patient can register in the new system once, without occupying other places.
At the same time, however, he did not hide the existence of technical problems, speed interruptions, which greatly slow down the reception and cause dissatisfaction because eHealth does not work at all.
According to the director of the Lithuanian Association of District Hospitals, Vygantas Sudaris, the systemic errors are also notable, as some of the registration hours do not coincide with the hours of the doctors themselves.
“Also, a patient who has not been registered with a specific GP can do so through IPR,” he said.
Many sign up to call themselves
Šiauliai Republican Hospital joined IPR IS in August and left sergu.lt entirely. Its director, Remigijus Mažeika, stressed that the institution had been waiting and preparing for him, but so far not everything is working as he would like.
“The system has resulted in a significant improvement in the quality of the shipments, as the paper shipments have been of very poor quality. However, there are other problems: the practice of registering patients for consultation has not changed.
The absolute majority, that is, 95 percent. the patients themselves, having a referral, call reception and look for vacancies. Only about 5 percent. patients register themselves through the IPR system. Therefore, in terms of efficiency, the situation has remained the same, and a large number of specialists are needed in the reception areas. People are calling, they are not happy and we still do not have a breakthrough in this place. I think these are long-term jobs, ”said R. Mažeika.
At the same time, he noted that sometimes the updated updates made by the institution and the Registration Center allow 2 patients to register for the same voucher.
Patients lack advertising
Both the heads of the institutions and the representatives of the patient organizations unanimously stressed that, among other things, there is a great lack of publicity that sergu.lt will no longer work.
Neringa Čiakienė, director of the Cancer Patients Association (POLA), noted that a large number of patients are the least aware of these changes.
The patient representative jointly made suggestions that would be relevant to the patients.
“The registration period must be standardized in all institutions. Because now it is possible to register with some specialists until the end of January, others have hours until June.
Another thing is that it must also be informed that this registration period will be renewed, new schedules will be given and again it is desirable that these terms are the same in the different treatment facilities, “he said.
Among other things, he emphasized the need to set up an automated SMS system that would confirm the registration immediately and later by sending reminders.
“This is especially relevant now that restrictions on movement between municipalities are applied during this period. And patients complain that in several cases they do not receive emails during the execution. And since there is a significant proportion of older patients who send emails . does not use mail, this would be the most convenient SMS sending system, “said N. Čiakienė.
For their part, SAM representatives confirmed that this functionality was already implemented and encouraged institutions to use it.
Due to the passivity of use, there is a lack of attractiveness.
In response to the above problems, D. Vilytė agreed that the possibility of connecting to both systems so far is misleading for consumers.
“It is not good, but the first quarter will come out of this situation. We are forced to create new functionalities of the system, services, improve the technical part in parallel. “Some updates have just been done, it should have been felt,” said a representative from the Records Center.
He stressed that it is a good and fair tool, only that until now due to the passivity of use it has not been so attractive for patients.
