The general contractor was China Construction Eighth Engineering Bureau Co ., Ltd., and the labor subcontractor Shanghai Xilaile Industrial Co., Ltd. is responsible for the primary and secondary structural engineering of the main body, and the supervisory unit is Hefei Kangda Engineering Consulting Co., Ltd. On March 13, This year, during the work of the masonry team of Shanghai Silile Industrial Co., Ltd., due to the breakage of the protective cover of the basement light shaft, three workers, Ma Mouyou, Zhang and Zhang, fell to the first floor of the basement. Ma was seriously injured and passed away after being taken to hospital. The other two injured had facial contusions and chest fractures, and were not in danger of death. Shanghai Xilaile Industrial Co., Ltd. reached a compensation agreement with the family of the deceased and paid the compensation in one lump sum. The “March 13” undercover accident investigation team of the second phase of the Maoyue Garden project believes that the direct cause of the accident was that the input of the light shaft did not perform safety protection in accordance with the regulations, and only the protection horizontal composed of wood and timber formwork. Shanghai Xilaile Industrial Co., Ltd. failed to fulfill primary responsibility for safety production and did not provide safety education and training to operators in accordance with regulations. There were gaps in the security management at the site of the Shanghai branch of China Construction Eighth Engineering Co., Ltd., and the investigation of hidden hazards at the site was not exhaustive and no light wells were found. Problems such as the demolition of the protection of the entrance facade are the indirect causes of the accident. The accident investigation team suggested that, in accordance with relevant regulations, Zhan X, the person in charge of Shanghai Xilaile Industrial Co., Ltd. Xu X and others, as well as Shanghai Xilaile Industrial Co., Ltd., China Construction Eighth Bureau Shanghai Branch, and other companies should receive administrative penalties.
