The Ministry of Health has published a statement of informed consent to be vaccinated against COVID-19. It will be signed by anyone who wishes to be vaccinated.
In it, the three names of the citizen, PIN, DNI and permanent address are entered. Consent can also be given by a parent, guardian, or administrator.
The statement establishes that the signer knows the benefits of the COVID-19 vaccine that has been selected and its commercial name and agrees to be given it.
We also give our consent to the appearance of side effects after vaccination, to report their appearance at any time to medical professionals or the Executive Agency for Medicines.
In the document the date of vaccination is entered, and the vaccinated person and the doctor who sets the dose put their signatures.
The Minister of Health has already issued an order with which vaccination will begin in our country on December 27.
Tomorrow, December 26, between 7:00 a.m. and 8:00 a.m., the first vaccines against COVID-19 will arrive in Bulgaria. This was announced by the press center of the Ministry of Health.
