The patient’s wound at the time of hospitalization, swelling, necrosis of the right hand, complications of blood clotting disorders, and severe thrombocytopenia caused the wound to bleed continuously.

According to the patient’s relative, on the morning of March 13, the patient went to cut lemongrass, unfortunately he was bitten on the middle finger of his right hand by a snake. After that, the patient did not go to the hospital, but to the traditional healer for the treatment of snake venom.

However, the wound became more and more swollen, accompanied by severe pain. Then blisters appeared, bleeding more and more.

One day after being bitten by a snake, the patient was taken to an emergency hospital and transferred to Can Tho Central General Hospital in critical condition.

The strange thing is that 4 hours later, a person who lives in the same neighborhood as patient T, Mrs. LTBB (52 years old), was also hospitalized at the Central General Hospital of Can Tho in the same situation of being bitten by a snake. .

After being bitten by a snake, Patient B. went home and then a medicine man wrapped the medicine. However, on the afternoon of the next day (March 14), when the condition of the purple arm and severe pain did not subside, the patient went to the local emergency hospital and was transferred to the Can Tho Central General Hospital.

Mr. Nguyen Chi Hung, son of patient B., said that on the afternoon of the same day, Ms. B., on her way home from work, immediately stopped by the lemongrass that patient T. was bitten by. a snake in the morning to remove the lemongrass. Not knowing that someone had been bitten by a snake before, Ms. B. also accidentally let the snake bite her left arm.

After being bitten by a snake, Mrs. B. went home and then the healer wrapped up the medicine. However, on the afternoon of the next day (March 14), when the condition of the purple arm and severe pain did not subside, the patient went to the local emergency hospital and was transferred to the Can Tho Central General Hospital.

At the time of admission, Patient B.’s left forearm was necrotic, swollen, extensive redness, and many blisters along with blood clotting disorder and systemic bleeding.

Immediately after receiving the previous 2 snakebite cases, the hospital transferred both patients to the Department of Intensive Care – Antivenom for active treatment according to hospital protocol; patients receive blood products such as red blood cells, platelets, and fresh frozen plasma for the treatment of coagulation disorder caused by snake venom.

The problem is that anti-eczema snakes are not available, so the hospital has to ask Cho Ray Hospital (Ho Chi Minh City) to provide support to quickly neutralize the snake venom for the patient. Both patients had to undergo dialysis treatment, an incision to relieve pressure in the compartment of the forearm of the hand that was bitten by a snake.

The patient with LTBB evolved very seriously, septic shock, but fortunately, after 3 days of intensive treatment, there was a positive recovery.

However, only patients with LTBB disease progress very severely, with septic shock, suspected by inflammation of the cellular tissue of the left forearm due to a snake bite; complications of blood clotting disorders, severe thrombocytopenia, gastrointestinal bleeding … Subsequently, the patient fell into a coma, vomiting fresh blood, low blood pressure, severe respiratory failure, and needed to be ventilated and equipped with a breathing device.

Fortunately, after 3 days of intensive treatment, the patient was able to stop the ventilator, remove the breathing tube, be awake, and have good contact.

By the afternoon of March 18, both diseases had stabilized, only patient T. was being actively treated for a blood clotting disorder and thrombocytopenia.

Snake bite is better immediately to the hospital.

This is a very dangerous poisonous snake that often causes accidents in the tropical countries of Southeast Asia. In Vietnam, indigo snakes are common in rubber and cashew growing areas in the southeast, south central, central highlands, Bay Nui (An Giang), and southern limestone regions such as Kien. Luong, Ha Tien belongs to Kien Giang Province.

Dr. Phuoc said that when people are accidentally bitten by a snake, people must adhere to first aid principles to slow down the absorption of snake venom into the body. Stop the patient from moving and place the snake-bitten limb under the heart to decrease the absorption of toxins. Then wash the wound; Bandage the affected limb with a bandage, starting from above the bite to limit the absorption of toxins along the lymphatic pathway.

“The next thing is to quickly transfer patients to the hospital. Folk medicine self-treatment is not advisable because such medical conditions (such as skin incision, oral poisoning, or breathing to smoke poison …) are not recommended and can cause infection, increase absorption of poison, and bleeding. All snakebites should be monitored in the hospital for the first 24 hours, at least 12 hours, ”said Dr. Phuoc.
