On the evening of September 21, MSc.BS Phan Anh Dung – tDepartment of Gastrointestinal Unit, Department of General Internal Medicine, Thu Duc District Hospital (Ho Chi Minh City) – said the male patient with TVT (24 years old, living in Ho Chi Minh City) had to give emergency immediately coughing up blood massively, the blood flows in clots (also called “coughing up blood from lightning “).
Before that, Mr. T. used to cough up blood 2-3 times a day in small amounts. I suspect that I am tuberculosis, went to the Thu Duc District Hospital to examine him.
Here, doctors discovered that Mr. T. suffered a lot of lung damage due to tuberculosis, causing him to cough up blood. A week later, Mr. T. went to the hospital for a new exam. then Suddenly “coughing up blood and lightning” right in the hospital bed, critical, possibly fatal if there is no timely intervention.
Doctors quickly resuscitated the initial session and activated the internal red alert procedure and interdisciplinary consultation to provide a plan to save the patient. Mr. T. later were quickly referred to the vascular intervention room to treat bleeding due to arterial rupture, simultaneously endotracheal intubation, invasive mechanical ventilation in the circuit intervention room.
After an hour of intervention, Mr. T. he had survived the critical attack and was intubated the next day. He is currently discharged from the hospital, but needs to continue monitoring, taking anti-tuberculosis drugs for 6 months and a new examination to consider intervening in the sequelae of tuberculosis after treatment for tuberculosis.
Before that in August, Thu Duc District Hospital also intervened to stop the bleeding and provide timely emergency care. patients with VTTT (65 years) cardiac arrest, complete apnea from “coughing up blood from lightning.”
The doctors said “coughing up blood from lightning” is a sudden and ultra-fast disease, the blood can spit out massively and cannot be stopped, the bleeding turns into clots blocking the airways, after a few minutes will lead to acute respiratory failure, insufficiency circulatory system and possibly death immediately afterwards, cannot keep up with the doctors back hand.
Common disease in patients with lung problems such as tuberculosis, in this case, it is a rare accident but has a very high mortality rate, over 90%.
