– This morning, the Viet Duc and Cho Ray hospitals will hold a consultation to assess the possibility of lung transplantation for patients 91.

Patient 91, 43 years old, the Vietnam Airlines pilot is currently the heaviest Covid-19 patient under treatment in Vietnam.

The Treatment Subcommittee, the National Steering Committee for Prevention and Control of Covid-19 said that this morning the Viet Duc Hospital and the Hospital for Tropical Diseases in Ho Chi Minh City will join the National Coordination Center for transplantation of human organs. Interhospital consultation to evaluate the possibility of lung transplantation for this patient.

Currently, a patient’s test results for diphtheria are positive for SARS-CoV-2. The patient is in a very critical condition because the right lung has collapsed in the lower lobes, the left lung solidifies and contracts, infection increases, liver enzymes increase, coagulopathy.

Patients continue to be treated with antibiotics, antifungals, mechanical ventilation, and ECMO cardiopulmonary support.

Professor Nguyen Gia Binh, a member of the professional council, said that if you want to receive a lung transplant for a patient, 91 you must wait until the patient’s lung condition is free of infection. In addition, there are many other factors that depend on the source of the lungs, donor and recipient compatibility …

In Vietnam, there are 3 facilities that perform lung transplants, in which Hospital 103 implants lung from living donors, Hospital 108 and the Viet Duc transplant Hospital of donated brain-dead donors, the Viet Duc Hospital only performs 2 cases .

According to professional advice, the patient’s immune system overreacted when it attacked the SARS-CoV-2 virus, causing the body to release more cytokines against the body itself, causing internal effects.

This is a medical phenomenon called cytokine release syndrome or cytokine storm, but the cause cannot be explained.

Because the patient was resistant to all coagulant medications used in the country, the Ministry of Health had to order rare medications from abroad to treat this male patient.

Until now, the Ho Chi Minh City Hospital for Tropical Diseases continues to pay the full treatment fee for 91 patients.

Patient 91 was admitted to hospital on 3/20 with a right parenchymal lesion, after which health remained poor, high fever.

The patient received supportive treatment to breathe oxygen through the nose, then switched to oxygen to breathe through the mask from 3/25. Ten days later, the patient continued with invasive mechanical ventilation due to severe progression of the disease. As of 6/4, patients with high fever, respiratory failure, must endure ECMO and last from then until now. As of 7/7, patients with multiple organ failure have many prognostic stages that are difficult to overcome.
