Two teenagers and a man in his 20s died after the car they were traveling in collided with a tree near Dudley in the West Midlands.
They were among the five people in a yellow Skoda Fabia that crashed in Bromley Lane, Kingswinford, around 8:50 pm Tuesday.
An air ambulance, an intensive care vehicle, three paramedics, an emergency doctor and six ambulances were dispatched to the scene.
A teenage boy and a teenage girl are in critical condition at Queen Elizabeth Hospital in Birmingham.
The man, who was driving, and the two girls, who were sitting in the back, were pronounced dead at the scene, West Midlands police said.
The force said officers had seen the vehicle moments before the crash, but did not speak to the occupants or pursue it.
The families of everyone involved have been informed.
Confirming that the highway had reopened after being closed overnight, Sergeant Dean Caswell said the investigation was in an early stage.
He said: “This is a tragic time for the families of those involved, and we have specially trained officers to support them in the difficult time ahead.”
“I urge anyone who has seen the Skoda driving in the area last night, or who has captured something on the dash cam footage, to contact us as soon as possible.”
A spokeswoman for the West Midlands Ambulance Service said: “Crews arrived at the scene and found that a car had hit a tree, with five patients involved.
“Unfortunately, it soon became clear that nothing could be done to save the driver of the car, a man, and he was pronounced dead at the scene.
“Despite the best efforts of the staff, the deaths of two teenagers in the back seat of the vehicle were also confirmed.
“Our thoughts are with the families of everyone involved at this time.”
