PITTSBURGH (KDKA) – After announcing that bars are closing again and that restaurants cannot serve alcohol in Allegheny County, both the Pennsylvania Department of Health and Allegheny County Health Department have released online forms for Inform companies that do not follow the order.
This occurs after Allegheny County reported the most positive cases of COVID-19 in Pennsylvania for the first time during the Coronavirus pandemic.
On the county website, the Food Service Department requests their contact information and a brief message of the situation.
You can access the web form on their website or email them to [email protected].
As for the Pennsylvania Department of Health, the form goes a bit deeper and asks for contact information, business information, and the nature of the complaint.
You can access the state’s COVID claim form on their website.
