The HDC investigation found that while the woman signed a consent form, the form was “rudimentary and non-specific” and did not adequately indicate the risks and complications of the procedure.
It also found that the ophthalmologist violated Law 4 (1) of the Code of Rights of Consumers of Health and Disabled Services by failing to ensure that the correct treatment package was given before starting the surgery, and by using the package when it was the wrong size. .
