The Minister of Health, Roberto Speranza, had already presented Italy’s strategic plan for the coronavirus vaccine in Parliament. Now the presidency of the Council of Ministers, together with the Ministry of Health, the ISS, Aifa and Agenas publishes a document that puts in black and white the different phases of the vaccine plan. First of all, let us remember that Italy has joined the initiative of the European Union to purchase a package of vaccines from various pharmaceutical companies. Vaccine doses, which will then be distributed to the different Member States based on population, will be available from January 2021. The plan highlights the different levels of coordination of the vaccination campaign, already described as the most impressive they are never faced. The different administration points will be coordinated at the local level and will have as reference the Region, which in turn will be able to count on the national coordination of the Ministry of Health, in which the extraordinary commissioner for the coronavirus emergency will also participate.
The vaccine distribution model
The first doses to arrive will likely be those of the Pfizer-BioNTech vaccine and then those of Moderna. However, most of the vaccines purchased at the European Union level, and therefore in proportion also the majority of those that will reach Italy, are those from Astra Zeneca, as can be seen in the following table.
The vaccination plan will be developed in several stages, corresponding to the number of doses available. Initially, health workers, guests of nursing homes and people over 80 will be vaccinated. Then it will be the turn of those over 60, the categories at risk based on the clinical picture, teachers and school personnel. Essential service workers and high-risk workers, such as those who work in prisons and communities, will then be vaccinated. Finally, it will be the turn of the rest of the population.
How many doses will depend on each region?
Take the Pfizer vaccine, the first vaccine likely to be available. For this you will need special thermal boxes, since the doses must be stored at -70 °, which are found in hospitals. Have already been identified 277 management points and it is estimated that 1.8 million people can already be vaccinated in the first phase: 70% of healthcare personnel and 30% of RSA guests.
At the regional level, as can be seen in the following table, the number of people to be vaccinated, the administration points (distinguishing between those available and those currently communicated) and the provinces that do not have the special thermal cells to store the vaccine. Therefore, it was highlighted that some Regions are proceeding with the purchase of the necessary ULT freezers for which no reimbursements are foreseen, while others (or individual ASLs) are launching tenders independently. Interlocutions in these cases are ongoing.
It is also announced that the manufacturer will send the doses to a single national center where the vaccines will be stored and preserved: for Italy, it will be the military airport of Pratica di Mare. The ALS will then proceed to request the amount of doses necessary and everything will be coordinated by the extraordinary commissioner.
How to reserve for the vaccine
But how can you reserve it for the vaccine? Can be done via an app or the toll-free number national. “The reservation is made simultaneously for the two vaccination dates.“It is specified, remembering that for each vaccination two doses must be administered. All the necessary data must be entered at the time of reservation, to expedite the procedures later in the acceptance phase. For RSA everything will be managed through a call center. On the scheduled date you will have to present yourself at the administration point with an identity document. The vaccine will be administered by a doctor or nurse or even by a health assistant. Then a surveillance period will have to be passed so that, in In case of reaction, the doctor can intervene.
The call for medical personnel
For each point of administration, the goal is to have a doctor and 4 nurses, while a doctor and a nurse will be sufficient for home administration. It is estimated to be able to do 6 vaccines every hour (3 if the administration is at home). These are the numbers defined to guarantee the administration of 60 million doses, which will be enough to 30 million people, in 7 months. Clearly, it will be necessary to strengthen the available health personnel, so in the coming days the special commissioner will launch two calls: one for doctors and nurses and another for administration companies. The call, as shown in the table below, may participate in graduate doctors qualified for the exercise of the profession and registered in the professional order, nurses and health aides registered in professional orders, retired doctors and nurses, scholarship holders, observer personnel . administrative.
