Certificate of no impediment to the entry or return of a school. It is the certificate that the family pediatrician or general practitioner will issue after taking care of the patient, verifying the cure. After taking two swabs (which must be negative) 24 hours apart, one from the other. The Ministry of Health, with a circular signed by the General Director of Prevention Giovanni Rezza, regulated the “certificates of recovery of Covid-19 or of a pathology other than COVID-19 for students / school personnel suspected of having Sars-CoV-2 infection. ‘
In particular, Rezza’s indications refer to four scenarios, which are combined to define a “suspected case”, also based on the evaluation of the treating physician. The first and second cases refer to a student who “has an increase in body temperature above 37.5 ° C or symptoms compatible with COVID-19 at school or at home “; the third and fourth cases when a school worker is in the same conditions at school or at home.
And it is at this point that the ministry creates a preferred lane for students, teachers, and administrative staff to clean up. Faced with suspicious symptoms, the treating physician must request the diagnostic test and report it to the Prevention Department (DdP) that performs the diagnostic test.
If the case is confirmed, the DdP takes steps to deepen the epidemiological investigation. If the test is positive, the DOP of the case is notified, which initiates the search for contacts and indicates the extraordinary actions to clean up the school structure in its interested party. “To return to the community it will be necessary to wait for healing according to current criteria,” the circular reads. Scientific indications foresee two tampons at a distance of 24 hours from each other with a simultaneous double negative, which can be achieved by returning to the community, through a certificate of cure.
If a student or school worker cohabits with a case, they will be considered a close contact and will be quarantined after evaluation by the Prevention department. “Any close contact of him (for example, classmates of the student in quarantine), does not require quarantine, unless the Prevention department makes further evaluations after a positive diagnostic test in the close contact of a case.”
The document must put order and put an end to the difficulties of the directors during these first days. “At this moment it is chaos – denounces Cristina Costarelli, vice president of the National Association of Deans of Rome – Doctors stiffen, contesting what for us are suspicious cases. Unfortunately, we must also count on the negatives in the issuance of health certificates. We also had to deal with phantom local health authorities with whom we communicated only by email. Even doctors now have to organize according to the new guidelines published yesterday. Family doctors sound the alarm: the system is unable to withstand a wave of widespread tampons. “We are doing our part – says Paolo Biasci president of the Italian Federation of Pediatricians – but it is not possible to have to wait four or five days or even longer for the result of the swab when, if the same patient goes to the emergency room, the answer arrives in four hours. It is essential to reduce the disparity between hospital hours and the territory. The alternative, health professionals very clearly warn, is precisely chaos.
Ten days after the reopening of the school year, it is estimated that more than 400 institutions have already been affected by at least one case of Coronavirus and 75 have been closed. The worst affected regions, Lombardy, Emilia Romagna, Tuscany and Lazio. Among the cities with the most difficulties, Rome with 19 schools involved.
