Quick swabs, stop by the family doctors: we can’t do them, we need courses. 22 thousand unused kits

It will be very difficult for the citizens of Puglia to make quick swabs (those based on the research of the virus protein) in the offices of free choice family doctors and pediatricians. And most importantly, antigen testing is unlikely to begin anytime soon. Yesterday in the Region the table was established that, after the signing of the contractual agreement at the ministerial headquarters (signed by the two great acronyms, Fimmg and Fimp), must define the complementary agreement that will establish how and where the buffers will be made. And the matter is complicated.

Apulian local health authorities have already received (or are about to receive) the first batch of about 22 thousand kits containing antigenic swabs, a cotton swab (to be passed on the tongue or in the nose) which is then inserted into a bottle containing a reagent: change in color indicates any positivity. They should be used for students to ensure their recovery and allow them to return to school, thus relieving public health services. This, at least, provides the Acn (not free: the doctor will charge 18 euros per swab).

Three days ago ASL Bari sent a Pec to all family doctors asking them to indicate where they would like to receive the kit. And the doctors didn’t like the rush. Discontent emerged yesterday at the regional table, confirming that it will not be easy to define the protocol and activate everyone, especially the unions that have not signed the ACN.

Doctors (all) say in fact that it is risky to take swabs in private consultations, and they ask for a public center in each municipality where they can take turns, one day a week. For the Region, however, this is an impractical solution: the rapid test must be performed within a day or so, so the client would have to wait for the daily shift of the respective family doctor. However, the department head Vito Montanaro has already activated the conversations with the ANCI: in many municipalities, tents and drive-throughs can be used, where they exist. In the larger centers (eg Bari) the territorial structures of the Asl can be used. The real problem is time.

“The times do not depend only on us”, says Nicola Calabrese, general secretary of Fimmg. “The objective,” he explains, “is to provide the service to all citizens, but also to protect personal situations: there are colleagues who are pregnant or with particular weaknesses, for whom exemption and substitution are foreseen. The general medical system will be in charge of supporting them. A new technical meeting is scheduled for Tuesday in which the operational proposal to bring back to the table will be defined (at least it should). “We have a binding agreement for everyone because we have to make a swab”, acknowledges Luigi Nigri, FINP regional secretary (pediatricians). The real problem is deciding where.

“Pediatricians have unanimously decided to use dedicated structures, creating dedicated centers at the municipal level for those who have to go back to school.” However, with an argument shared by his colleagues, Nigri asks that “before starting a training course an understanding of how the tests are carried out should be made. Otherwise, we could make a lot of mistakes.
Meanwhile, the Region should close the new Covid hospital plan today. The last consultation with the heads of the bed and the health directors of the company is scheduled for midmorning.

Tomorrow morning the list of public and private hospitals accredited with the new beds will be ready, divided between Intensive Care, Infectious Diseases and Pulmonology, as well as post-covid. The Region has followed the scenarios developed by the group of epidemiologists: the planned beds will go “far beyond” the 2,900 hospitalizations planned at the end of the month.[m.scagl.]
