With the spread of variants of COVID-19Starting with English, which is already responsible for more than 50% of contagion cases nationwide, there are new indications for prevention and containment. From the increase in physical spacing at 2 meterswhenever possible and using evidence «multi-geni» to identify mutations. But also to quarantine the vaccinated, if «close contact» and regardless of whether with one or two doses. The recommendations are contained in a new report prepared by Inail, Iss, Aifa and the Ministry of Health.
At least two meters away
For physical distancing, it is the indication of the Report «Provisional indications on measures for the prevention and control of SARS-CoV-2 infections in terms of variants and vaccination», one meter is still the minimum distance to adopt but it would be advisable to increase it “up to two meters, when possible and especially in all situations in which respiratory protection is removed, such as when consuming drinks and food”.
Furthermore, “it is not indicated to modify prevention and protection measures based on the use of masks and hand hygiene; on the contrary, an extremely careful and rigorous application of these measures is considered necessary ”.
Quarantine also for the vaccinated
Another clue concerns close contacts. In fact, it is highlighted that even those who are vaccinated against Sars-CoV-2, after a high-risk exposure with a Covid case “must adopt the same preventive indications valid for a person not subjected to vaccination, regardless of the type of vaccination” . vaccine received, number of doses and time elapsed since vaccination ”.
The vaccine considered in close contact must observe, whenever it is asymptomatic, 10 days of quarantine from the last exposure with negative antigen or molecular test on the 10th day or 14 days from the last exposure. Close contacts of a Covid-19 case, on the other hand, can be vaccinated but “must complete the 10-14-day quarantine before they can be vaccinated.”
New evidence
Regarding the diagnostic tests to be performed, to guarantee the diagnosis of infection supported by viral variants with mutations in the Spike protein, “real-time PCR molecular diagnostic tests must be multi-target”, that is, capable of detecting more genes of the virus and more the spike gene (S) that could give negative results in case of variant. The document then confirms the vaccination indications for the recovered subjects. People with prior Sars-CoV-2 infection confirmed by molecular testing, regardless of whether they have symptomatic Covid-19 or not, “should get vaccinated.”
“A single dose can be considered as long as vaccination is done at least 3 months after infection and within 6 months of infection.” Exceptions are people with immunodeficiency diseases, primary or secondary to pharmacological treatments, who, even if they have a previous Sars-CoV-2 infection, “should be vaccinated as soon as possible and with a two-dose vaccination cycle.”
Also vaccinated subjects, the Report also warns, “although with reduced risk, they can suffer infection by Sars-CoV-2 since no vaccine is 100% effective and the immune response to vaccination can vary from one subject to another. Furthermore, the term of protection has not yet been defined. ‘ It is emphasized that the risk of reinfection by Sars-CoV-2 was assessed in a multicenter study of more than 6,600 healthcare professionals in the UK. The results show that “in subjects with previous Sars-CoV-2 infection the probability of symptomatic or asymptomatic reinfection is reduced by 83%.” Furthermore, in subjects with previous Sars-CoV-2 infection “the duration of the protective effect of the previous infection has a median of 5 months”.
