Are the provisions of the Memorandum of Understanding signed today, November 5, by the health minister, Simone Bezzini, and by Alessio Nastruzzi, general secretary of the Italian Federation of Family Physicians (Fimmg), in the presence of the President of Tuscany, Eugenio Giani.

“It is a very important act, a preliminary agreement with doctors, which is the highest and noble expression of teaming up in a time as complex as the current one – Giani declares – We are facing a pandemic that has never been seen since the Spanish flu onwards. The emergency we are experiencing requires extraordinary commitments. The role of family doctors in the area is fundamental. They are an integral part of our social and health system and together with they will be able to guarantee that assistance that does not congest our hospitals ”.

“Health surveillance, contact tracing and improving care for patients with suspected symptoms of Covid 19, activation of ASL continuity assistance centers, are the consultation areas, on which he had place a close and collaborative discussion between the parties, just in recent days, with the specific desire to counteract the spread of the virus and protect the health of citizens as much as possible – adds Commissioner Bezzini-. The agreement implements one of the indications of the ordinance of October 24 on the improvement of traceability through the implementation of quick swabs also with the participation of family doctors, who will be able to do the screening not only in their studies or at home, but also in organized outpatient structures, and in case of microfocolai situations. In addition – Bezzini continues – it allows us an experimental innovation on the continuity of the healthcare or primary care service, aimed at freeing up resources and personnel for follow-up or outpatient activities for diagnostic purposes only or in a health hotel always on a voluntary basis ”.

The agreement provides, on an experimental basis, throughout the period of the pandemic emergency, that the Local Health Authorities, through specific company agreements, implement the h16 care model, which aims to imply the continuity of care in support of the activity for the Covid 19 emergency. The service will operate through the continuity assistance centers during the night (from 20 to 8 hours). The hours of night activity of the doctors outside the Center’s activity will be reused during the day for the management of the pandemic emergency. The medical service will remain unchanged, in all currently active offices, until midnight (including Saturdays and Sundays). On weekends, Saturdays and Sundays, the Central may request buffers to attend tracking requests, thus ensuring the continuity of the service.
Under this Agreement, family physicians will be able to operate in a structure, including mobile, configured by the ASL in walk-in / drive-through mode, subjecting both patients who require an antigen test to the swab at the end of the 10 days of isolation by close contact, both patients suspected of having had close contact. In case of need, even the screenings carried out with rapid swabs (or another suitable system in a situation of “microfocolaio” declared by the competent authority) can be guaranteed by family doctors also within their territorial functional aggregations (Aft) or District Zone / SdS, according to the organizational methods agreed with the corresponding Local Health Authority.

“Today’s pre-agreement allows us to strengthen the regional health system and contribute even more incisively in the fight against Coronavirus – adds Alessio Nastruzzi, regional secretary of Fimmg -. We are a team and we must try to work in an organized way. At this particular time, it is more crucial than ever to help each other. Family doctors have a privileged relationship with our clients, with whom we are in constant contact. Cleaning those who need it should be understood as an extension of our diagnostic capacity. We really can do a lot, working on the close contacts of positives and thus helping to rebuild the infection supply chain. We are available – Nastruzzi concludes – to allocate resources where they are most needed, for now on an experimental basis. Finally, the protocol defines, among the commitments of the Region and the ASL, in addition to the identification of suitable spaces, also the supply of tampons and personal protective equipment for the performance of the activity by family doctors.
