How to cure Covid-19 at home? How to be treated by family doctors? After months of waiting, the draft of the protocol of the Ministry of Health has arrived, which changes the cards on the table with respect to the previous guidelines: at home we should not resort to antibiotics and the use of cortisone is not recommended. “We were stuck with the indications for March and April,” said Silvestro Scotti, head of the Fimmg union, the largest union of family doctors. But the strong protest comes from the Federation of General Medicine. The secretary of Piedmont, Roberto Venesia, speaks for it and attacks: «To treat Covid patients at home, you need a homogeneous protocol of use, defined tasks, clear functions. The guidelines should be written in collaboration with virologists and pulmonologists. It is also necessary to distinguish between therapies for people under 50 years of age and for patients over 50 ”. According to the March indications, Scotti explained, the patients were treated with paracetamol, ibuprofen and vitamins, in the first instance, and if the fever persisted, antibiotics and cortisone were used. Now, however, we read in the draft that for home treatments, doctors should use “paracetamol for febrile symptoms, anti-inflammatory if the clinical picture of the patient with Covid begins to worsen” while cortisone “is recommended only in case of emergency to avoid assaulting the patient “. immune system. “The guide also warns against” not using any antirheumatic and, above all, antibiotics. “Heparin, on the other hand, is recommended for people who have difficulty moving.”
The new indications
The new indications for family doctors have been developed by a working group of the Ministry of Health that also includes Franco Locatelli – member of the Scientific Technical Committee – with the aim of alleviating the pressure of too many patients in hospitals. “But if dyspnea occurs and pulse oximetry drops too low, we indicate hospitalization,” explains Silvestro Scotti. Yours is a category under pressure. Now the 45,000 family doctors have also agreed to swab and identify positives and negatives among their patients. “We couldn’t hold back,” says Fiorenzo Corti, Fimmg’s national communication chief. And he adds: “We are fighting, but citizens have the right to have our care as general practitioners.” Citizens would also have the right to have doctors equipped with the appropriate tools, starting with pulse oximeters that often not even GPs can have in their studies.
Criticalities
Another open front is that of oxygen, which is essential to combat Covid-19. Therefore, it would also be essential to find it for sale in pharmacies, but it is becoming more and more complicated. Here is the cry of alarm: “We are facing a very serious shortage of oxygen cylinders, useful precisely for home treatments”, says Andrea Mandelli, president of the Federation of Italian pharmacists orders. “A situation of difficulty for oxygen therapy is being created throughout Italy – he continues – which, it must be emphasized, is the first and indispensable aid for Covid patients, but also for many others who suffer from chronic respiratory diseases.” The request from the pharmacists is very precise: «In these hours we receive many reports of cylinder shortages in various Regions, from Lombardy to Campania. Therefore, although pharmacists are doing everything possible to recover empty cylinders as during the closure, we ask all citizens to help us overcome this emergency: once exhausted, the cylinder must be returned promptly to the pharmacy, so that it can be disinfected, refilled and then made available to other patients who need it.
