School, in the classroom after the group test: here is the plan to monitor infections among students

From the saliva of children of a class will start the search for coronavirus. To speed up the controls, all the students will deposit the saliva in a single container, which will be examined. The result will arrive in 20 minutes. If there is no virus (as it will obviously happen in most cases) you will go to other classes without wasting too much time, if on the contrary there is a trace of SARS-CoV-2, then you will do the buffer for each individual student.

The Ministry of Health has already sent a request to the Scientific Technical Committee: rapid saliva tests can be used as schoolThose that quickly respond and allow analyzing the presence of the coronavirus in groups, therefore, with a speed that does not exist with other systems? The CTS is analyzing the different systems, but the orientation that now transpires, from several fronts, is to use this type of tampon in a massive way.

They are proving to be reliable and are dramatically reducing waiting times compared to serological or molecular swab testing. Also at the Spallanzani Institute in Rome, salivary swab controls are being carried out and will conclude at the end of the month. Caution is mandatory, but the first results seem very reassuring. For schools it will be a revolution because saliva tests speed up times a lot, as Professor Massimo Galli, from Sacco in Milan, also explains. Why does everything become faster and less invasive? Suppose there is a positive child in a school. At that time, antigenic swabs are made to the colleagues, with a very banal saliva sample and the results are obtained in 15-20 minutes. Not only that: in the other classes, as a precaution, saliva can be collected in groups and checked, in a few minutes, for the presence of Sars-CoV-2. If the coronavirus does not appear in the group of tubes of classes A, B, C, D, E and F, that is where the checks end, if instead it detects its presence in the class G tube, then at that point we proceed with the swabs in each individual student. What is the benefit of saliva? There are several: if we talk about children, compared to the quick swab in which a toothpick is inserted in the nose, it is less annoying; Furthermore, the fact that it can be analyzed by groups considerably increases the possible coverage.

Evolution

In fact, the saliva test is an evolution of the rapid antigen swab that is already being used at Fiumicino airport for tests on travelers arriving from Greece, Spain, Malta and Croatia and that will be used from Wednesday for the passengers departing on two Alitalia flights Rome-Milan. It is possible that in the coming days the controls will also affect those who return from France. But beyond the air travel front, the ability to perform quick swabs (nasal or salivary) represents the turning point in the peaceful management of the school year. The fear is that the epidemiological services of the ASL, as of today, will all be diverted to the controls of the classes whenever a suspicious case appears, impoverishing the monitoring of other outbreaks. But why is serological testing not used as it was before the school year began with teachers and school staff on a voluntary basis? The validity of this tool has been questionable: it is useful to understand the spread of the virus in a specific professional category, less to prevent the spread of the infection.

Just a few examples: in Veneto, out of 1,120 positive serological teachers, only 3 were actually found to be infected on molecular swab verification; the result was similar in Lazio, where 1.2 percent of the teachers were serologically positive, but only a dozen were infected, as the swab showed. In short: to understand, in a timely manner, if there is someone infected in a class, serological is not very useful, while traditional molecular is too slow. The focus is on the rapid antigenic swab, even better if the salivary is validated. Commissioner Domenico Arcuri explained that a public offering request was taken to purchase several million antigenic buffers. It will be a school year characterized by tests, but not written and oral. Those of the coronavirus.
