Whistleblower behind Gujarat HC order: ‘Not PPE kit, mask, gloves… can be a big problem’

ALL COVID patients are managed by young medical residents, without a professor in the service rounds, or emergency. The doctors do not Covid service are not given PPE kits, N-95 masks or gloves. Not even 10 percent of the nearly 700 medical residents, who stay in a hostel, have been tested.

These are some of the main charges that appear in the anonymous letter that is a part of the Gujarat high court’s scathing order in the poor Covid management and the high mortality rate in the state of the first line in the installation — Ahmedabad Civil Hospital.

The contents of the letter suggest that it was probably written for a period of 25 years of medical professional in the hospital gynecology department who were tested positive, while in the non-Covid duty.

In its order on Saturday on a suo motu public interest litigation, the High Court called the hospital, who has represented 377 Covid deaths (45% of Gujarat toll), “as good as a dungeon, it can be even worse.”

Describing the letter’s content as “very disturbing”, the court asked whether the Minister of Health and the Chief Secretary, had “some idea” about the problems faced by the patients and the staff.

On Sunday, the Health Minister, Nitin Patel said that the state government “has also appreciated the order of this Court and its observations” and “held a meeting to discuss and give an answer… next week.”

The letter, meanwhile, the details of the alleged inhuman conditions in the hospital so that the doctors and the claims that the settlements in the slums that poses a threat to the life of the patients.

“The address of the hospital is just a question of whether the residents taking the test, and if they prove to be positive, then, who is going to work. No senior teachers comes in rounds, or emergency.

All patients are managed by junior residents only. The management is not taking action for the residents, instead they are calling us cowards and kamchor (lazy). That is so that is not the case, especially when we are risking the lives and do everything we can despite all the odds,” he says.

The letter states that “eight residents of my department and five residents of the unit were tested positive and we were criticized for getting our testing done.” “Since five days, almost more than 30 residents are here, isolated in the Marriott Hotel. No one from the administration of the hospital has bothered to at least call us by our condition,” says the letter.

“Sir, we were having a great load of patients. Despite that, we were neither given EPI kit, or N-95 masks. Even appropriate gloves not available to conduct a normal delivery,” she says, adding that “excuses were given” in its place.

When I was contacted by The Indian Express for a response to the allegations raised in the letter, Dr M M Prabhakar, Officer of Special Duty (OSD) in the hospital, said: “Since it is a High Court matter, I’m not authorized and I can’t comment on this.”

The letter also makes reference to the alleged lack of evidence to the doctors in the hospital. “We are assuming that if the residents taking the test, half of the residents again, to be positive. Sir, imagine the chaos that is going to be caused. This is an urgent situation that requires urgent action,” he says.

They claim that it is difficult to work in the not-COVID duty, the letter states that despite the fact that many of the residents test positive in the same unit and department, the others have not been tested or their follow-up contacts.

“They are not understanding that the COVID +ve, if we work with patients who could not COVID is the duty of the patients affected by the virus, rather than cure. In the day that was given to me positive, I’ve done 3 Normal deliveries and a C-section, and almost came in contact with 20 patients directly and their newborns. There has been no contact with the track made by the same. No authority has been informed about it,” he says.

And in what seems to be a clue about the author, the letter states: “At the age of 25, nothing much will happen to me. I turned out to be negative within 5 days without any treatment, but that is not the same as that for pregnant women and newborns. Are immunocompromised that is to say, if we’re going to find with the virus, which can be a big problem.”
