- Tembisa Hospital Executive Director Dr. Lekopane Mogaladi faces suspension following a damning report from the Health Ombudsman on the death of Shonisani Lethole.
- The Gauteng health department issued Mogaladi a preventive suspension notice on Friday.
- The ombudsman found that the hospital was negligent in treating Lethole.
Dr. Lekopane Mogaladi, the embattled CEO of Tembisa Hospital, received a preventive suspension notice following a damning report from the Health Ombudsman Professor Malegapuru Makgoba regarding the death of Shonisani Lethole.
The Gauteng health department provided Mogaladi with the notice on Friday.
Department spokesperson Kwara Kekana said that the notification of the intention to place Mogaladi on preventive suspension was fulfilled in terms of Chapter 7 of the 2003 Senior Management Services Manual, as a result of the health ombudsman report on Lethole’s death at Tembisa Hospital in June last year. .
“As part of standard work practices, Mogaladi has been given the opportunity to make statements within five business days of receiving notice, regarding the intent to place you on preventive suspension.
“The department wishes to reiterate its commitment to implement the recommendations of the health ombud report on the circumstances surrounding Lethole’s care and death at the hospital within the legal prescriptions,” Kekana said.

The preventive suspension notice follows a scathing report released earlier in the week, which revealed that the hospital was negligent in treating Lethole.
Lethole died after being deprived of food for more than 100 hours.
Makgoba discovered that the hospital did indeed leave Lethole to die.
Among other damning revelations, Makgoba also revealed that the hospital was not truthful and provided incomplete information to the ombud.
Lethole’s death was inaccurately recorded and he died without knowing his Covid-19 status.
